Planning a Care Move: Seven Questions Before You Choose

A care move often begins with a long list of places and a short amount of time. Before choosing a room, pause to understand what the person needs, what the provider can support, and how the transition will work. A thoughtful plan covers moving out as well as moving in.

These seven questions can help families turn a tour into a useful conversation.

1. What support is needed throughout the day?

Describe a typical morning, afternoon and night. Ask about help with bathing, dressing, meals, mobility and medication routines. Share current assessments through an appropriate secure process. Ask the provider to assess the person’s needs before treating an opening as a confirmed fit.

2. What does this setting actually provide?

Oregon licenses and inspects adult foster homes, nursing facilities, assisted living and residential care settings, including settings with memory care. You can review public records through the Oregon Licensed Long-Term Care Settings Search. Use those records alongside a conversation about the specific location. A familiar label does not answer every question about staffing, services or daily life.

3. Who will be available when help is needed?

Ask who provides care during the day, overnight and on weekends. Who is the primary contact? How are changes in care needs handled? If a particular clinical service is needed, ask who provides it, how often they are present and whether outside services would be involved. Request clarification rather than assuming that an administrator’s credential means continuous on-site clinical coverage.

4. What will the total cost include?

Request a written explanation of the base rate, additional care charges, deposits and other fees. Ask when rates may change and what happens if support needs increase. If Medicaid or another payer is part of the plan, ask about the specific arrangement and what still needs confirmation. A general statement that a provider accepts a payer is a starting point for questions.

5. Is the opening suitable and available?

Confirm the room type, expected availability date, accessibility and any admission requirements. Ask how long the provider can hold a room and whether any payment or agreement is required. Availability can change between a tour and a move, so reconfirm before making arrangements.

6. How will the move itself work?

Agree on who coordinates transportation, belongings, medication information and communication between the current and receiving care teams. Identify one family contact and one provider contact. Ask what must be completed before arrival and what support is available during the first few days.

7. What if another transition becomes necessary?

Ask how the provider communicates changes, what happens when needs exceed its capabilities, and how move-outs are coordinated. Review the written agreement and ask for explanations of notice requirements, charges and responsibilities. Keep a record of unanswered questions so nothing gets lost in a busy conversation.
