A tour lets you see a care setting and start a conversation. Before making a choice, add public provider records to your questions.
Start with Oregon's Licensed Long-Term Care Settings Search at https://ltclicensing.oregon.gov/. Check that the provider name and street address match the place you are considering.
The search includes inspection reports, substantiated violations, and regulatory actions. Read the available findings and their dates. A short label or a single entry does not tell you everything you want to know about a person's day-to-day experience.
Make a small question list as you read:
• What happened, and when?
• How was the concern addressed?
• What changed afterward?
• What still needs clarification?
Bring those questions to the provider and take notes on the answers. Keep the record, the conversation, and the person's needs together as you compare options.
Public records are one part of the picture. They don't replace a current assessment of whether a home can meet someone's needs.
